Jayapura, which is the capital of Papua, has an unresolved TB problem. Table 1 shows the TB case discovery rate/Case Detection Rate (CDR) in 2015, which was 547 cases. In 2015, the screening rate for suspected pulmonary TB was found to be 1,463 per 100,000 population. The proportion of BTA positive patients was 17% of suspects, the proportion of BTA positive pulmonary TB patients who were treated was 44%, and the proportion of pediatric TB patients among all TB patients was 14%. Meanwhile the conversion rate was 69%, and the cure rate for Jayapura City in 2015 was 68%. (Source: Head of the Immunization Section of the Jayapura City Health Service, 5 August 2017). In 2016, the number of TB cases discovered was 207 and mid-2017, 15 drug-resistant TB (MDR) patients were found (Source: SR TB Aisyiyah data). The highest recovery rate was 68% which was achieved in 2011 and 2015, far below the national recovery target of >85%.

TB disease caused by the bacteria Mycobacterium tuberculosis has a negative socio-economic impact on society, especially on families or people around TB patients. A patient will need money to carry out TB treatment, the duration of treatment takes 6-24 months. Patients with TB (regular and MDR) are estimated to cost IDR 12 million to IDR 68 million per patient. This amount is used for costs for undergoing treatment, living and family needs excluding drug costs. Another condition that the patient will face is loss of income for himself and his family. If calculated from the length of treatment with the regional UMK, a loss of IDR 16-64 million will be found per patient.
Health matters are a concurrent matter of mandatory basic services that must be implemented by local governments. Based on Law Number 23 of 2014 concerning Regional Government, regional governments are responsible for human resource matters and community empowerment in the health sector. Apart from that, Minister of Health Regulation (Permenkes) Number 43 of 2016 Regarding Minimum Service Standards (SPM) in the health sector, it is stated that "Every person with TB receives TB services according to standards and must be served 100%". Thus, the Jayapura City Government has an obligation to implement TB control efforts in accordance with the established SPM which is reflected in its policies and budget.
